Thrombomodulin is an endothelial cell-surface glycoprotein that binds thrombin and activates the protein C anticoagulant pathway. Soluble TM fragments shed into the circulation during endothelial injury, making TM a widely accepted biomarker of vascular damage, coagulation activation, and inflammation in preclinical monkey models of sepsis, atherosclerosis, and disseminated intravascular coagulation.

Researchers commonly pair TM measurements with other coagulation markers such as D-dimer, protein C, and thrombin–antithrombin complex to characterize endothelial dysfunction in cynomolgus or rhesus macaque studies.
