ITIH3 belongs to the inter-alpha-trypsin inhibitor (ITI) family, where it acts alongside other heavy chains (ITIH1, ITIH2) and the light chain bikunin to form covalent complexes that modulate protease cascades during the acute-phase response. The ITIH3 gene maps to chromosome 3p21.1, a region associated in genome-wide studies with susceptibility to BRCA1/2-linked breast cancer. Altered circulating ITIH3 concentrations have also been reported in hepatocellular carcinoma, ovarian cancer, and chronic inflammatory conditions, making the protein an active target in biomarker discovery programs.
